Insurance
Kathryn accepts most major insurance plans. Services may be covered in full or in part by your health insurance or employee benefit plan.
Good Questions to Ask Your Insurance Company and Consider
Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per year?
What is the coverage amount per session?
Is approval required from my primary care physician?
Payment
All payments are due at the time of service. A credit card number and deposit is required to hold the initial session. Kathryn will go over her fees with you.
Cancellation Policy
A credit card is required with a deposit on the client's account prior to any consultation. This includes clients that are to be placed on the waiting list. This is to protect both the client and therapist if client fails to show for the initial consultation or violates the cancellation policy. It also secures your place in line on the waiting list. Thereafter, it will remain in the clients account with the understanding that the full deposit will be lost if the client fails to schedule final exit session with KMT at least two weeks prior to leaving.
*Please note, clients who cancel and/or miss 3 consecutive sessions will be reviewed for termination of treatment or placed on the waiting list. To be determined on a case-by-case basis and at the sole discretion of Kathryn Miller.
Late for Appointment
It is very important to arrive on time for your appointments. If you are more than 15 minutes late, it will be considered a missed appointment.
