Good Questions to Ask Your Insurance Company and Consider
Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per year?
What is the coverage amount per session?
Is approval required from my primary care physician?
All payments are due at the time of service. A credit card number and deposit is required to hold the initial session. Kathryn will go over her fees with you.
A credit card is required with a deposit on the client's account prior to any consultation. This includes clients that are to be placed on the waiting list. This is to protect both the client and therapist if client fails to show for the initial consultation or violates the cancellation policy. It also secures your place in line on the waiting list. Thereafter, it will remain in the clients account with the understanding that the full deposit will be lost if the client fails to show or does not cancel 48 hours prior to the scheduled appointment time. If deposit is then lost and client violates the cancellation policy a second time, the full amount of the session will be charged to the card on file. Although the office does send out reminders, it is still the patient's responsibility to keep track of appointments. Patients also will have access to their own patient portal where they can view their appointments.
*Please note, clients who cancel and/or miss 3 consecutive sessions will be reviewed for termination of treatment or placed on the waiting list. To be determined on a case-by-case basis and at the sole discretion of Kathryn Miller.
Late for Appointment
It is very important to arrive on time for your appointments. There is a waiting area available in office should you arrive early. If you are more than 15 minutes late, it will be considered a missed appointment.
